Before your in-home visit with MindVet: Mobile Veterinary Behavior Services in Raleigh, NC, clients must review and acknowledge our policies and consent documentation. This form outlines safety expectations, permissions, and liability information for mobile veterinary behavior services provided to dogs and cats across the Triangle Area.

Please carefully review and complete this document prior to your scheduled mobile appointment. The form includes service policies, safety requirements—including securing your dog upon arrival—and consent for veterinary evaluation and treatment within the home setting.

Payment is due in full at the time of services rendered. All deposits are non-refundable. We accept all major credit cards and Care Credit. A $30 service charge will be applied to any returned payment.
